WELCOME TO FORT WORTH BRAIN & SPINE INSTITUTE, LLP
You have been scheduled for an appointment with one of our physicians. Please complete the patient forms on the home page and bring to your appointment. If your insurance requires a referral to see our physicians, please check with your primary physician to see that a referral has been made. You must have your completed packet (on the home page), insurance card(s) and driver’s license with you at the time of your appointment. If you do not have this information with you or your referral has not been received, your appointment may have to be rescheduled. You should plan to arrive 30 minutes before your appointment in order to complete the registration process.
For Dr. Alford, Dr. Bechtel, Dr. Ellis and Dr. Hubbard:
IF YOU ARE SEEING ONE OF THE ABOVE PHYSICIANS FOR AN INITIAL CONSULTATION, YOU MUST BRING ALL ACTUAL X-RAY FILMS (No CDs accepted) AND X-RAY REPORTS, EMG AND ANY OTHER TESTS THAT HAVE BEEN PERFORMED AS A RESULT OF YOUR PROBLEM. IF YOU DO NOT BRING YOUR ACTUAL FILMS (NO CDS), YOUR APPOINTMENT WILL BE RESCHEDULED. Please make every effort to have all films and reports with you in order to avoid rescheduling.
AGAIN, X-RAYS (MRI & CT) MUST BE FILMS. APPOINTMENTS WILL BE RESCHEDULED IF X-RAYS ARE ON A DISC.
For Dr. Lapsiwala and Dr. Siadati:
If you are seeing one of the above physicians, you are requested to bring actual films to your appointment. It is essential that you bring all the testing and films you have been requested to bring and it is preferred that you bring actual films but CD’s will be accepted on a case by case basis.
For Dr. Haque and Dr. Lee:
If you are seeing one of the above physicians, you are requested to bring CD format films to your appointment. It is essential that you bring all the testing and films you have been requested to bring and it is preferred that you bring CD’s.
PLEASE REVIEW THE POLICY FOR YOUR SPECIFIC PHYSICIAN BEFORE YOUR APPOINTMENT
If you are unable to make your appointment, please call our office as soon as possible so that we may schedule another patient in your time slot. We will reschedule your appointment at the next available time slot that is agreeable with you. Our office hours are 8:00 am to 5:00 pm, Monday through Friday. Please be aware that because our doctors are on call for emergencies, they sometimes run late seeing patients. Occasionally an emergency may necessitate canceling office appointments on short notice. In which case, we will make every effort to contact you.
Our office visits range from $180.00 to $440.00. If you are not a part of a PPO or HMO with which we participate, full payment for your consultation is due at the time of your appointment. It is your responsibility to verify with your insurance company prior to your appointment that our doctors are on your plan.
Our doctors participate with Medicare. You will be responsible for your deductible and twenty per cent co-pay at the time of your visit unless you have supplemental insurance.
If your injury is a liability claim, your visit here and any surgery will be on a cash only basis. Any surgery must be paid in full prior to entering the hospital. We do not accept letters of protection from attorneys.
Requests for prescription refills must be called in at least 24 hours prior to needing them refilled. No refills will be made over the weekend or holidays. Refills called on Friday afternoon will be called in to the pharmacy on Monday. Refills are given under the direction of the physician who reserves the right to refuse a refill at any time.
Our office is located on the eighth floor of the Harris Professional Center at the corner of Pennsylvania and Fifth Avenue. Parking is available in the adjacent parking garage. Please allow an extra 15-20 minutes for parking in the garage and coming to our office. One parking ticket will be validated when you check out.
